Terms and Conditions for Private Consultations

1. Introduction

These Terms and Conditions (“Terms”) apply to all private consultations and private clinical services provided by Forest Dispensing Chemist (“we”, “us” or “our”). By booking an appointment, attending a consultation, or receiving any private service with us, you confirm that you have read, understood, and agree to these Terms.

2. Nature of the Service

Private consultations provided by Forest Dispensing Chemist (Forest Chemist) are not NHS services and are offered on a self-funded basis. You are paying for the clinician’s professional time, assessment, and advice delivered by Forest Chemist. Outcomes cannot be guaranteed and any prescription, referral, investigation, or treatment will only be provided if clinically appropriate and at the clinician’s professional discretion.

3. Fees and Payment

All private consultation fees at Forest Dispensing Chemist (Forest Chemist) must be paid in advance or at the time of booking unless agreed otherwise in writing. The fee covers the clinician’s time for assessment, record-keeping, and advice. If a medicine, test, vaccination, or product is supplied by Forest Chemist, it may be charged separately unless clearly stated as included at the point of booking. Once a consultation has taken place, fees are non-refundable because the service (professional time and expertise) has been delivered by Forest Dispensing Chemist.

4. Cancellations, Rescheduling and Refunds

You may cancel or reschedule your appointment with Forest Dispensing Chemist (Forest Chemist) at least 24 hours before your appointment time without penalty. Cancellations within 24 hours or failure to attend (DNA) are not eligible for a refund or credit. If Forest Chemist must cancel or rearrange your appointment due to unforeseen circumstances, we will offer an alternative time or a full refund if no suitable alternative can be arranged.

5. Clinical Outcomes, Safety and Liability

Clinical outcomes vary between individuals and cannot be guaranteed by Forest Dispensing Chemist (Forest Chemist). We are not liable for lack of improvement, non-response to treatment, or differences of opinion between healthcare professionals. If you experience side effects or have concerns following treatment provided by Forest Chemist, contact us promptly using the contact details above. If symptoms are severe or worsening, seek urgent medical help via 999 or NHS 111.

6. Patient Responsibilities

You agree to provide accurate and complete information to Forest Dispensing Chemist (Forest Chemist) including medical history, current medicines, allergies, pregnancy or breastfeeding status where relevant, and any recent changes in health. You must follow clinical advice and aftercare instructions given by Forest Chemist during or after the consultation. If you do not disclose relevant information, it may make treatment unsafe or unsuitable and Forest Chemist cannot be responsible for harm arising from missing or inaccurate information.

7. Eligibility, Consent and Capacity

Some services offered by Forest Dispensing Chemist (Forest Chemist) have minimum age requirements or require a capacity assessment. Where appropriate, we will ask for consent before proceeding, and consent may be withdrawn at any time. If a service is not suitable due to age, clinical risk, or safeguarding concerns, Forest Chemist may decline to provide it and recommend an alternative route of care.

8. Confidentiality and Data Protection

Forest Dispensing Chemist (Forest Chemist) handles personal data in line with UK GDPR and the Data Protection Act 2018. Clinical records are confidential and stored securely. Information may be shared with other healthcare professionals where needed for your care or where required by law.

10. Governing Law and Jurisdiction

These Terms are governed by the laws of England and Wales and any disputes will be subject to the exclusive jurisdiction of the courts of England and Wales.

11. Acceptance

By proceeding with a booking or attending your appointment at Forest Dispensing Chemist (Forest Chemist), you confirm that you have read and understood these Terms, you accept that consultation fees are non-refundable once the consultation has taken place, and you understand that no guarantee of outcome or prescription is implied.
